• FOLLOWING THE SUCCESSFUL EXECUTION OF A BAA (IF NECESSARY) AND A DATA USE AGREEMENT (DUA) THROUGH THE STANFORD UNIVERSITY CENTER FOR POPULATION HEALTH SCIENCES (STANFORD PHS), WHICH SERVES AS THE DATA CUSTODIAN FOR ELECTRONIC HEALTH DATA AND MEASURES-RELATED DATA WITHIN THE PRIME REGISTRY, PROVIDE THE AIR TEAM WITH ACCESS TO THE AMERICAN FAMILY COHORT (AFC) DATASET (ALTERNATIVELY KNOWN AS THE PRIME REGISTRY EHR DATA), AFC MEASURES-RELATED ANNUAL DATA, AND AFC NOTES. PRIME CONTRACTOR AGREES AND ACKNOWLEDGES THAT ABFM’S PERFORMANCE UNDER THIS SUBCONTRACT IS CONTINGENT ON THE EXECUTION OF A DATA USE AGREEMENT AND ANY NECESSARY BAA BETWEEN STANFORD AND PRIME CONTRACTOR.
