| Subcontractor | Department of Health and Human Services | PROJECT OBJECTIVE:
THE PROPOSED RESEARCH WILL TAKE ADVANTAGE OF A LARGE (N=6,012), WELL-CHARACTERIZED COHORT OF AFRICAN AMERICAN WOMEN WITH LONGITUDINAL ASSESSMENTS OF CERVICAL LENGTH BEGINNING IN THE MID- TRIMESTER AND THE COLLECTION OF BIOLOGICAL MATERIAL TO PERMIT GENOTYPING OF MOTHERS FOR THIS PROPOSAL. FOR FUTURE STUDIES, BIOBANKED SAMPLES OF OFFSPRING DNA, CERVICO-VAGINAL FLUID FOR THE MICROBIOME, AND OTHER BIOMARKERS INCLUDING CYTOKINES ARE AVAILABLE. THE COHORT ALSO INCLUDES 83 WOMEN TREATED WITH VAGINAL PROGESTERONE DUE TO A PREGNANCY COMPLICATED BY A SHORT CERVIX. THE KEY COMPONENT OF THE PROPOSED PROJECT WILL BE A GENOME-WIDE ASSOCIATION STUDY (GWAS) OF MATERNAL DNA FOR LONGITUDINAL ASSESSMENT OF ULTRASOUND-MEASURED CERVICAL LENGTH AS A QUANTITATIVE TRAIT. THE FINDINGS OF THE DISCOVERY GWAS WILL BE VALIDATED BY TARGETED GENOTYPING OF AN INDEPENDENT AFRICAN-AMERICAN COHORT. THE VALIDATED LOCI WILL THEN BE USED TO ASSESS HOW SELECT GENOTYPES CAN MODIFY PROGESTERONE RESPONSIVENESS.
WORK BEING PERFORMED:
SUBCONTRACTOR WILL BE RESPONSIBLE FOR THE CONTINUED DEVELOPMENT, IMPLEMENTATION, AND INTERPRETATION OF THE LONGITUDINAL MODELING FRAMEWORK AND ANALYTIC PLAN. SPECIFICALLY, THE WORK WILL INCLUDE (1) FINALIZING THE PRETERM BIRTH (PTB) RISK MODEL, (2) EXTERNALLY VALIDATING PROSPECTIVE ASSOCIATIONS IDENTIFIED BETWEEN THE GROWTH PARAMETERS FOR CERVICAL LENGTH (CL) AND PTB OUTCOMES, (3) DEVELOPING AND PILOT TESTING A MODEL TO FORMALLY ASSESS MEDIATION OF GENOMIC INFLUENCES (E.G., POLYGENIC RISK SCORES; PGX) ON PTB VIA PREGNANCY-SPECIFIC CHANGES IN CL, AND (4) EXTENDING THE AFOREMENTIONED MEDIATION MODEL TO TEST FOR MODERATION OF PGX ASSOCIATIONS (I.E., WITH CL GROWTH PARAMETERS AND PTB) BY SINGLE NUCLEOTIDE POLYMORPHISMS (SNPS).
DELIVERABLES:
QUARTER 1 €€Œ AUGUST 15TH, 2022: DEVELOPMENT OF A TESTABLE MODEL OF MEDIATION OF GENOMIC INFLUENCES ON
PRETERM BIRTH OUTCOMES VIA PREGNANCY-SPECIFIC CHANGES IN CERVICAL LENGTH
QUARTER 2 €€Œ NOVEMBER 15TH, 2022: PUBLICATION OF PROSPECTIVE LINKS BETWEEN PRE-PREGNANCY MATERNAL COVARIATES AND PRETERM BIRTH OUTCOMES, AND MEDIATION THROUGH PREGNANCY-SPECIFIC CHANGES IN CERVICAL LENGTH; PILOT TESTING OF MODERATED-MEDIATION MODELS.
