THIS PROPOSAL AIMS TO DEVELOP THE APPLICATION OF THE ZINC IONOPHORE PBT2 TO DISRUPT THE ANTIBIOTIC RESISTANCE MECHANISMS OF STREPTOCOCCUS PNEUMONIAE, HAEMOPHILUS INFLUENZAE, AND MORAXELLA CATARRHALIS, THE PRIMARY ETIOLOGICAL AGENTS OF COMMUNITY ACQUIRED BACTERIAL PNEUMONIA (CABP). PBT2 IS A SAFE-FOR-HUMAN-USE HYDROXYQUINOLINE IONOPHORE THAT WAS ORIGINALLY DEVELOPED TO TREAT HUMAN NEURODEGENERATIVE DISEASES AND PROGRESSED TO PHASE II CLINICAL TRIALS. WE HAVE REPURPOSED PBT2 TO FACILITATE THE INTRACELLULAR ACCUMULATION OF ZINC IONS WITHIN BACTERIAL PATHOGENS AND SHOWN THAT THIS DISRUPTS THEIR ABILITY TO RESIST TREATMENT BY BETA-LACTAM AND TETRACYCLINE CLASS ANTIBIOTICS. IN THE CARB-X SUPPORT PROJECT WE WILL DEMONSTRATE THE BREADTH OF ANTIBIOTIC RESISTANCE BREAKAGE BY PBT2 AGAINST A PANEL OF GLOBAL STRAINS, GENERATE FURTHER EFFICACY DATA FOR PBT2+ANTIBIOTIC TREATMENT IN MOUSE MODELS OF LUNG INFECTION, UNDERTAKE A PRELIMINARY PHARMACOKINETIC STUDY, AND PERFORM A GAP ANALYSIS OF THE PRECLINICAL AND CLINICAL DATA FOR PBT2.

DR KATHERINE KEDZIERSKA'S LABORATORY WILL DIRECT EX VIVO DETECTION AND PHENOTYPIC/FUNCTIONAL ANALYSIS OF INFLUENZA-SPECIFIC CD8+ T CELLS AND CD4+ T CELLS USING PEPTIDE-HLA CLASS I AND CLASS II TETRAMERS, WHICH WE WILL PROVIDE FOR THE STUDY. WE WILL PROVIDE MONOMERS, TETRAMERSIZE THEM, OPTIMIZE MULTI-PARAMETER TETRAMER PANELS TOGETHER WITH MULTI-COLOUR FLOW CYTOMETRIC PANELS FOR PHENOTYPIC AND FUNCTIONAL ANALYSES. SHE WILL WORK CLOSELY WITH DR PAUL THOMAS'S GROUP TO DETECT DIRECTLY EX VIVO INFLUENZA-SPECIFIC CD8+ AND CD4+ T CELLS BY TETRAMER ENRICHMENT IN HUMAN SUBJECTS. THIS WORK WILL BE GIVEN THE HIGHEST PRIORITY. DR KEDZIERSKA'S LABORATORY HAS BEEN RESEARCHING HUMAN T CELL IMMUNITY TO PANDEMIC, SEASONAL AND NEWLY EMERGED INFLUENZA VIRUSES, ANTI-VIRAL IMMUNITY IN THE YOUNG, THE ELDERLY, AND INDIGENOUS AUSTRALIANS, AS WELL AS GENERATION OF IMMUNOLOGICAL MEMORY FOLLOWING INFLUENZA EXPOSURES OVER THE LAST 10 YEARS. THUS, THIS WORK RELATES CLOSELY TO MY RESEARCH INTERESTS

TASK/PROJECT SCOPE: THE SCOPE OF THIS TASK IS TO UNDERSTAND MECHANISMS UNDERLYING DISEASE SEVERITY AND POOR VACCINE EFFICACY IN THE SOUTHERN HEMISPHERE TO INFORM RATIONAL DEVELOPMENT OF EARLY BIOMARKERS FOR INFLUENZA DISEASE SEVERITY AND IMPROVED THERAPEUTIC INTERVENTION FOR PATIENTS AT RISK OF DYING. TO ACHIEVE THIS, WE WILL RECRUIT PATIENTS HOSPITALIZED WITH INFLUENZA DISEASE IN OUR WELL-ESTABLISHED RESPIRATORY DISI COHORT (AIM 1) AND RECRUIT TOP END INDIGENOUS AUSTRALIANS INTO OUR WELL-ESTABLISHED LIFT/LIFT-V COHORTS AT A ROYAL DARWIN HOSPITAL AND A REMOTE INDIGENOUS COMMUNITY IN THE NORTHERN TERRITORY (AIM 3). IN OUR DISI COHORT, WE WILL COLLECT LONGITUDINAL BLOOD SAMPLES AND NASAL SWABS IN PATIENTS WITH SUSPECTED INFLUENZA-LIKE ILLNESS (ILI). INFLUENZA-POSITIVE PATIENTS ARE PCR-CONFIRMED, WHILE ILI INFLUENZA-NEGATIVE PATIENTS POSITIVE FOR OTHER RESPIRATORY DISEASES ARE TREATED AS CONTROLS. BLOODS ARE COLLECTED WITHIN 24-72 HOURS OF HOSPITAL ADMISSION (VISIT 1, V1), EVERY 2-5 DAYS UNTIL DISCHARGE (V2, V3, V4 ETC.), THEN FOLLOWED UP A30 DAYS LATER. OUR WELL-ESTABLISHED LIFT COHORT REPRESENTS ADULT INDIGENOUS AUSTRALIANS, EITHER HEALTHY DONORS OR PATIENTS WITH ILIS (LIFT), OR INDIVIDUALS RECEIVING SEASONAL INFLUENZA VACCINE (LIFT-V). WE HAVE LONGSTANDING RESEARCH RELATIONSHIPS WITH A REMOTE ABORIGINAL COMMUNITY, WHERE WE HAVE INVESTED IN CAPACITY BUILDING LOCAL COMMUNITY-BASED ABORIGINAL RESEARCH ASSISTANTS, INCLUDING 4 WHO HAVE COMPLETED A FORMAL CERTIFICATE 2 IN COMMUNITY-BASED RESEARCH THROUGH CHARLES DARWIN UNIVERSITY. THIS COMMUNITY WITH A POPULATION OF 2500-3000 IS A TWO-HOUR FLIGHT FROM DARWIN AND HAS A CLINIC, WHICH PROVIDES PRIMARY HEALTH CARE; UNWELL PATIENTS ARE EVACUATED TO ROYAL DARWIN HOSPITAL (RDH). WE WILL RECRUIT INTO 2 ARMS (1) HEALTHY INDIGENOUS AUSTRALIANS RECRUITED IN A REMOTE ABORIGINAL COMMUNITY; (2) HEALTHY INDIGENOUS AUSTRALIANS RECEIVING SEASONAL INFLUENZA VACCINATION RECRUITED IN A REMOTE ABORIGINAL COMMUNITY, AS ABOVE.. WE WILL HARMONIZE OUR STUDIES WITH HUMAN COHORT 5 (INDIGENOUS POPULATIONS IN COLUMBIA) SO THE RESULTS CAN BE ANALYZED TOGETHER.
